End of Life Team

We provide specialist support to patients and their families who wish to have end of life care at home. 

Who is the service provided to?

Adults (18 and over).

Where is the service provided from?

In the North East Essex Community.

When is the service provided?

Seven days a wekk

8.30am  5.00pm

How do people access the service?

You will normally need a referral from your GP, Community Nurse or Hospice Nurse Specialist (for patients in the community).

Patients in Hospital can be referred by the Ward or Hospital Discharge Team.
