Crisis Response Service

This service is an integrated health and social care Crisis Assessment and Response service. The aim is to provide immediate care and management of any patient who is experiencing a health or social care crisis, who might otherwise need admission to hospital.

The service is offered to patients whose care needs can be safely met in the community.

Who is your service provided for?

This service is targeted primarily at people over the age of 65 who are registered with a North East Essex GP.
