Community Rehabilitation

The Community Rehabilitation Service for North East Essex provides assessments and treatments for patients in their place of residence. 

The service is a short term service offering multifactorial assessment and intervention.

The services include:  Physiotherapy and Occupational Therapy.

The aim of treatment is to support you to improve your own  functional ability and independence using self management techniques. This may include an exercise programme, the provision of equipment, advice and signposting to support you in managing your own condition.

Who is the service provided for?

The service is provided to the adult population (16+) for North East Essex.

Who delivers the service?

We have a team of Physiotherapists, Occupational therapists, Associate practitioners and Assistant practitioners, supported by our Nursing and Admin teams.

Who will visit me?

This will depend on who has the best skills to be able to support you through your Rehabilitation

How do you access the service?

Referrals are accepted from Social Care, hospital medical staff, nurses and GPs via the Community Gateway.
